[ Change of address ]
If you change name, address or telephone number, please let our receptionists know by calling or writing to us. There are occasions when we may need to contact you regarding your healthcare, sometimes urgently. If you move outside the practice area you may need to find a doctor in your new area. There are instructions on what to do when you move to a new area on the back of your NHS medical card.
[ Medical education ]
We are a training practice and you may be offered an appointment with a GP registrar. These doctors are fully qualified will have had experience in several hospital specialties. They are completing their specialist training as a GP.
We also teach medical students and sometimes you may be asked to see the doctor or nurse with a student present. You will always be asked beforehand if this is acceptable and you may ask for the student to withdraw if you prefer to have your consultation in private.
[ Disabled access ]
The surgery is fully accessible to disabled people. To gain wheelchair access, please ring the front doorbell and a member of staff will guide you to the ramp access door. There is a designated parking bay adjacent to the surgery entrance.
[ Complaints policy ]
Granton Medical Centre aims to give a friendly and professional service to all our patients. However, if you have any concerns about any aspect of our service, please let us know. Speak to whomever you feel most comfortable - your GP, our practice manager or our reception staff will be happy to help. In the majority of cases concerns can be resolved quite easily.
We take complaints very seriously. If you would like to make a complaint regarding the surgery or the services we offer, please contact the Practice Manager by telephone or, if you prefer, in writing. Every effort will be made to answer your concerns as soon as possible.

The PCT also operates a Patient Advice and Liaison Service (PALS), which can often help resolve any problems before they become formal complaints. If you wish to speak to a PALS officer, ring 0121 442 3311.
[ Access to medical records ]
The practice is registered and complies with the Data Protection Act 1998. Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act. Please contact the Practice Manager for further information.
[ Patient confidentiality ]
We respect your right to privacy and keep all your health information confidential and secure. It is important that the NHS keeps accurate and up-to-date records about your health and treatment so that those treating you can give you the best possible care.
This information may be used for management and audit purposes. However, it is usually only available to, and used by, those involved in your care. You have the right to know what information we hold about you. If you would like to see your records please contact the Practice Manager.
[ Patient rights and responsibilities ]
We aim to treat our patients courteously at all times and expect our patients to treat our staff in a similarly respectful way. It is your responsibility to keep your appointments, inform us of your past illnesses, medication, hospital admissions and any other relevant details.
[ Violent or abusive behaviour ]
We take seriously any threatening, abusive or violent behaviour against any of our staff or patients. If a patient is violent or abusive, they will be warned to stop their behaviour. If they persist, we may exercise our right to take action to have them removed, immediately if necessary, from our list of patients.

[ Telephone Advice ]
The most convenient time to speak to a doctor is after morning surgery, ie around 11.30pm - 12.30pm. If the doctor is still seeing patients you will be asked to leave a number where the doctor can reach you. We would advise you not to go out if you are expecting the doctor to call you back.
[ NHS Care Records Service]
The NHS in England is introducing the NHS Care Records Service (NHS CRS). Over time, the NHS CRS will begin to provide healthcare staff with quicker access to reliable information about you to help with your treatment, including in an emergency.
GP practices in South Birmingham Primary Care Trust are amongst the first in England to begin implementing the new NHS Care Records Service as part of an 'Early Adopter programme'. 'Summary Care Records' are being created for South Birmingham patients.
Existing patients of Granton Medical Centre will already have received information about the service. Newly registering patients will be given information when they attend the practice to register. Patients can limit their participation and also opt out of having a Summary Care Record. If you choose to opt out, you need to visit surgery to speak with a receptionist and sign the necessary form.

Extended Hours Appointments
The purpose of Extended Opening Hours is to offer appointments to patients who are not usually able to attend surgery during the core opening hours due to work commitments etc. Since July 2009 we have been offering these appointments on Tuesday and Thursday evenings between 6.30pm and 7.30pm and on Tuesday and Friday mornings between 7.00am and 8.00am. However feedback from patients indicates that there is a preference for the availability of Saturday morning appointments. Therefore, with effect from 8th May 2010 we will be offering the Extended Opening Hours on Saturday mornings between 8am and 12.00pm. The surgery will be open for these pre-booked appointments only. THERE ARE NO OTHER SERVICES AVAILABLE, receptions and phone lines are CLOSED. The surgery is locked and access will only be given to patients who have pre-booked an appointment with a doctor. Anyone requiring a doctor on the day can attend the Selly Oak Walk in Centre at Katie Road, Selly Oak or alternatively ring the out of hours service on 0845 113 0004.
[ Registering for online services ]
We now offer online GP appointment booking and repeat prescription requests. This is in co-operation with EMIS, our computer system supplier and is called 'EMIS Access'.
To register for this service, you must attend the surgery and bring a proof of identity with you. For example, driving licence, passport or a proof of address in the form of a current utility bill. You will be given a registration letter from our receptionist who will go through the EMIS Access guidance with you.
Once you have a registration letter, create your account with EMIS Access here. You will be required to use a password to set up your account. It is your responsibility to keep your password safe and to remember it. The practice will not have access to your account and if you forget your password you will no longer be eligible to be registered for EMIS Access.
